Certificates of Insurance

The City of Niagara Falls requires vendors and contractors to provide proof of insurance before starting any contract, project, or special event.


Certificate of insurance forms

Click the links below to access the City's three insurance forms: Standard, Professional Liability, and Contractor Certificates of Insurance. Your insurance provider must complete and sign these forms, which are the only certificates accepted by the City.

Standard liability form

This form is for general agreements, contracts, permits, and purchase orders where proof of liability insurance is needed. For example, if you are renting a city facility for an event or a contractor doing work for the city, you would use this form to show you have insurance coverage.

Professional liability form

This form applies to professionals like consultants or architects who offer specialized services to the City. It ensures they have insurance in case their advice or services cause a loss.

Contractor liability form

This form applies to official contracts that include the Niagara Peninsula Standard Contract Document, which sets rules and guidelines used for road and municipal construction projects in Niagara.  Do not confuse this with the Standard liability form that contractors working for the City must use.
